Unum Voluntary Supplemental Insurance Benefits

All Pavilions employees working at least 24 hours per week in a regular part time or full time position are enrolled in an employer paid $5000 life insurance policy. This starts the first of the month following 60 days of employment, or after changing status to an eligible position working at least 24 hours per week.

• Get more information on the coverage provided by the $5000 life insurance policy.
• Make sure you have completed the Beneficiary Designation form for your life insurance policy. Please turn in all completed forms to Human Resources.
