Instructions:
Enter your first vacation choice with optional start/end dates. Enter second request as alternative if first request is not granted.
Do not put different quarter dates on any one request, and do not submit two forms for the same quarter.
Each employee may be granted one vacation, based on seniority and staffing requirements.
If you request is denied, you may inquire regarding other available time frames.
Holiday: If your holiday to work falls during an approved vacation, you will still be scheduled for that date.
Weekends: If your approved vacation includes your normal weekend to work, you will be scheduled an alternate weekend.
Requests due as shown below:

Vacation approval/denial information will be emailed to you before Nov 15, May 15, Feb 15, Aug 15. (Copies in Scheduling for Nursing and Cottages or ES Office for ES Staff).
